PACK (Practical Approach to Care Kit) aims to equip health workers to help them manage all common adult conditions at a primary care level. It offers practice guides and implementation strategies for users; the starting point is any of 40 common symptoms, each of which provides the opportunity to identify one or more of 20 important chronic conditions in the second half of the guideline. These include infectious diseases, chronic respiratory diseases, cardiovascular risk, hypertension and diabetes, mental health conditions, musculoskeletal disorders, epilepsy and women’s health . It has been developed by the Knowledge Translation Unit on the University of Cape Town Lung Institute over a period of 12 years.

PACK Adult was implemented initially as Primary Care 101 in the Eden district in the Western Cape Province of South Africa as a part of a randomized trial of its effectiveness and was rebranded in 2012 to form part of the Practical Approach to Care Kit Package. This package includes a guide for community care workers (PACK CCW), patient information leaflets, PACK Child and PACK Counsellor.

In 2012, six PACK trainers trained 206 health workers in 76 facilities in the district. The training and programme development model has been regarded by the provincial Department of Health as best practice. The programme has since then been expanded to the rest of the province.
